The NASA chart of “Significant Incidents and Close Calls in Human Spaceflight” has been in development over the past several years under sponsorship from NASA’s Office of Safety and Mission Assurance. Originally published as hardcopy version is now available in interactive format. The chart remind us that in addition to well-known incidents several other events have happened in human spaceflight history that have had the potential to cause injury to flight crews or result in fatalities. The interactive chart provides detailed information on significant events in outer space and near-space (20-160 km) missions. It includes several features such as the ability to focus on specific programs or vehicles and even goes down to system level.
